First Time Client?
Please print out and complete the medical intake form (.PDF) shown below and bring it with you to your first session.
If you are a Groupon Client, please remember to bring your voucher with you.
Please note that my office does NOT have a waiting area while other clients are being seen, so to avoid unnecessarily waiting in either the elements or your vehicle, please try arrive no earlier than 10 minutes before your scheduled session. NOTE: Clients who do arrive significantly ahead of their scheduled time (greater than 10 minutes prior) may be asked to wait outside of the office.
I do my best to provide e-mail confirmation through Google Calendars Invite, but it is the client’s responsibility for keeping track of his/her own individually scheduled time.
Late-arriving clients / Late Cancellations / No-Call No-Shows
Please do your best to arrive on time. If you arrive late, I will do my best to fit in your entire session if my schedule permits. Otherwise, I will work for the remaining time and payment for your full session will be expected.
If you need to reschedule or cancel your appointment, please do your best to do so 24 hours in advance of your scheduled time. Those who do not may be subject to a cancellation fee up to the full value of their session. Emergency situations are always taken into consideration.
Clients who do not show up to their scheduled session without having advised me ahead of time will be asked to pay for the missed session. Groupon clients, your voucher’s value will be reduced to the actual cash paid for service.
